Your janma nakshatra is the lunar mansion the Moon was in at the moment you were born. Vedic astrology divides the sidereal zodiac into 27 nakshatras of 13°20' each, and each nakshatra into four padas of 3°20'.
It is one of the most used facts in a Vedic chart. It sets the order of your Vimshottari dasha periods, it is the basis of Ashtakoot kundli matching, it is used to pick auspicious dates, and each pada carries the traditional starting syllable for a name.
Your local birth time is converted to Universal Time with the time zone rules that applied on that date, including daylight saving and historical changes such as India's wartime clock. Swiss Ephemeris then gives the Moon's exact position for that instant.
That position is converted to the sidereal zodiac with the Lahiri (Chitrapaksha) ayanamsa, the standard adopted by the Government of India. The nakshatra is the 13°20' segment the Moon falls in, and the pada is the quarter of that segment.
The Moon moves about 13 degrees a day. That is roughly one nakshatra a day and one pada every six hours. If you were born near a boundary, a birth time that is off by an hour or two can change your pada, and sometimes your nakshatra.
If you do not know your birth time, enter your best estimate and treat the pada as uncertain. The result tells you how far into the nakshatra the Moon was, so you can see whether you are close to an edge.
When people say "my nakshatra" they mean the Moon's nakshatra, and that is the headline result here. The calculator also shows the nakshatra of your ascendant (lagna), which depends strongly on the exact birth time, and of your Sun, which changes about every two weeks.

Each nakshatra is split into four quarters of 3°20' called padas. There are 108 padas in the zodiac, nine in every sign. The pada refines the nakshatra and is used for name syllables and for the Navamsa (D9) chart.

The usual causes are a different ayanamsa (Raman's differs from Lahiri by more than a degree), a site that ignores daylight saving or historical time zones, or a Western tropical zodiac. Near a boundary, any of these can move you into the next pada or nakshatra.
